Recognition/Renewal of Students Organizations for AY 2014 – 2015

A T T E N T I O N !

The Office of Student Affairs (OSA) is now accepting applications of Recognition/Renewal of Students Organizations for AY 2014 – 2015.

The following are the requirements:

A. RENEWAL OF STUDENTS ORGANIZATION

a. Letter of application signed by the Head/President of the Organization and endorsed by the Faculty Adviser

addressed to the Dean;

b. Letter of acceptance of Faculty Adviser or Co-Adviser with address and contact number;

c. Strategic plan of activities for AY 2014 – 2015;

d. List of all officers and members with name, course and year, contact number, scanned recent pictures, and final

grades of the previous semester; and

e. Annual Accomplishment and financial report

B. NEW ORGANIZATION

a. Letter of application signed by the Head/President of the Organization and endorsed by the Faculty Adviser

addressed to the Dean;

b. Letter of acceptance of Faculty Adviser or Co-Adviser with address and contact number;

c. Strategic plan of activities for AY 2014 – 2015;

d. List of all officers and members with name, course and year, contact number, scanned recent pictures, and final

grades of the previous semester;

e. Annual Accomplishment and financial report; and

f. Copy of Constitution and By-Laws approved and attested by the Head/President of the organization noted by

the Adviser and Group interview of all officers at OSA on designated dates.

NOTE:

All documents should be submitted to OSA on or before 17 September 2014.
